Structured Family Caregiving

Do you live with and care for a loved one?
You could be their paid caregiver!

SFC (Structured Family Caregiving) is a Medicaid Waiver program that provides income to loved ones taking on the responsibility of caring for an elderly family member. SFC provides a high-quality alternative to care models that rely on facilities or in-home workers such as PCA's. The caregiver may be family (including spouses) or a friend, but must live in the same home as the care recipient.

Families participating in our SFC program will receive regular visits and support by our experienced Caregiver Coaches. Helping caregivers feel more confident and supported in their role as their loved one's primary caretaker is our priority!

HomeCourt Home Care's SFC program was created to support and empower family caregivers to provide quality care for their loved one in a way that best meets the needs of their family.

Participant Eligibility

To qualify for this program, you must: 

1. Be eligible for Medicaid

2. Need 24 hour, live-in care

3. Need help with one or more activities of daily living (such as bathing toileting, walking, transferring, dressing, eating, etc.)

Caregiver Eligibility

To qualify as a caregiver in this program, you must: 

1. Live with the care recipient

2. Be at least 18 years old

3. Pass a background check

4. Commit to being your loved one's primary caregiver

A caregiver can be family (including spouses) or a friend. A caregiver may work another job outside of the home but must agree that the care of their loved one is their top priority.

Support

Financial Support:

Caregivers receive a daily tax-free stipend for their commitment. The stipend typically varies from $907-$1223 per month depending on the level of care.

Ongoing Support: 

Home visits with one of our experienced Caregiver Coaches will empower caregivers to provide quality care for their loved one with confidence.
